Duke Homecare & Hospice Duke Homecare & Hospice offers hospice, home health and infusion services as well as serves as the home for the Duke Caregiver Support Program. Our Hospice team works closely with a patient's physician to provide comprehensive, individualized care in the comfort their home or at our inpatient hospice facility located in Durham. Our team of nurses, therapists, social workers, counselors, and chaplains help patients and their families manage advanced medical conditions. Duke Home Health provides services in a patient's home that include skilled nursing, physical therapy, occupational therapy, speech therapy, social service, home health aides, and pain and wound management. Our infusion team comes to a patient's home to administer intravenous medications, including antibiotics and chemotherapy, to children and adults. Our nurses, pharmacists, dietitians, and patient services coordinators work under the direction of a patient's doctor to monitor needs and treatment.

JOB SUMMARY
In compliance with Duke HomeCare & Hospice philosophy, goals, policies, mission, and vision, the Director of Patient Care Services (Hospice) is responsible for the provision of quality hospice care services, operational effectiveness and efficiency of the delivery of patient care. This responsibility includes overall management responsibility for operating the patient services department, including program planning, revenue generation, operational and regulatory analysis, financial analysis, problem resolution, personnel management and performance improvement. The position oversees the Hospice Home Care Program, Partners Program (contracted facilities), Services at Hock Family Pavilion (General Inpatient Facility), GIP Care in Contracted Acute Care Facilities, Volunteer Services, and Bereavement Programs
MAJOR JOB RESPONSIBILITIES
Administration
Directs the assigned departments/service lines and delegates authority/ responsibility to meet goals.
Informs the Senior Director of Clinical Operations of the ongoing needs and accomplishments of the department.
Ensures that reports required by regulatory agencies and internal departments are prepared and submitted within established timeframes.
Prepares and presents administrative reports to the Quality and Safety Core Committee.
Analyzes the above reports developing corrective action plans for deficiencies as well as focuses on the positive impacts being made within the department and adjusts processes appropriately.
Participates in certification, accreditation, and audit procedures, keeping abreast of Federal, local and state laws, and accrediting body guidelines which affect clinical professional practice and hospice care regulations and assures procedural changes are implemented so that the entity remains compliant.
Responsible for overseeing the Performance Improvement Program.
Develops and implements policies and procedures supporting the department's mission and strategically planning for the department's future relative to the vision, mission, and priorities of the organization.
Performs other administrative tasks as determined by the Senior Director of Clinical Operations.
Cultivates Managers and Assistant Nurse Managers through exemplary modeling; allowing them the opportunity for growth within their leadership roles.
Financial
Prepare, monitor and evaluate operating and capital budget for department services.
Monitor on-going revenue and expenses for department services.
Submit financial reports to the Senior Director of Clinical Operations, Director of Finance, and Associate Vice President as required.
Work collaboratively with other members of the management team in providing cost-effective and maximize the efficiency of operations.
Work collaboratively with the Director of Finance to ensure processes/procedures related to billing, reimbursement, case management and other third-party payor requirements are met in a timely manner.
Personnel
Ensures the supervision of timely and competent provision of:
Patient intake, assessments, and documentation
Patient care coordination
Clinical monitoring services
24 hour on-call
Fulfillment of all required documentation activities
Assessing and providing equipment and supplies to patients.
Other functions necessary to provide patient care.
Oversee the appropriate hiring of staff.
Ensure that orientation, staff training and development programs are in place and provided to all staff.
Ensure systematic evaluation of staff performance.
Ensure personnel policies and procedures are followed and monitored.
Planning
Develops and implements annual department volume statistics.
Assists in identifying and implementing new programs in conjunction with clinical staff of DUHS.
Establish and implement processes to assure the integration/coordination of all disciplines in providing day-to-day patient care services.
Performance requirements
Maintains knowledge of hospice services by reviewing literature and attending in-services, seminars, and conferences as budget allows.
Demonstrates knowledge of hospice services and familiarity with state and federal regulations affecting hospice in-home and inpatient programs and alternate site operations.
Provide effective leadership and management.
Effectively participates with staff in the accomplishment of organizational goals.
Demonstrates extensive knowledge, understanding and ability to work with multi-health systems.
Displays excellent communication skills
Required Qualifications at this Level
Education
Work requires completion of an accredited Bachelor's degree in a clinical field such as Nursing, Pharmacy, Physical Therapy, Occupational Therapy, Speech Therapy or a related field, with professional licensure or registration in the clinical field as required.
Master's Degree in a clinical area or Health Administration preferred.
Preferred Degree: Nursing
Experience
A minimum six years of clinical experience, including five years of management/supervisory experience in a health care setting, of which at least three years of that experience is in acute care or homecare.
Degrees, Licensure, and/or Certification
Current North Carolina licensure or ability to obtain as required by professional discipline.
Membership and participation in professional societies and organizations is desirable.
Knowledge, Skills, and Abilities
In-depth knowledge of homecare industry. Maintains knowledge by reviewing literature and attending in-services, seminars, and conferences.
Exhibits effective leadership and management in health care organizations.
Demonstrates extensive knowledge, understanding and ability to work with multi-health systems.
Knowledge of medicare and medicaid regulations, JCAHO, and DFS standards.
Computer skills including Word and Excel.
Excellent verbal/written communication skills and organizational/time- management skills.
