Whether you prefer the support and camaraderie of a residential setting or the autonomy and flexibility of delivering care and comfort in a home setting, we hire RNs, CNAs, LPNs, rehab/therapy professionals, clinical leaders and activity coordinators to be part of our exceptional team. What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:   Previous work experience providing direct patient care in outpatient, inpatient or inpatient rehab unit preferred. Prior use of electronic medical records (EMR), preferred  Degree in Physical Therapy required. Master's or Doctorate degree in Physical Therapy, preferred Current Colorado state licensure as a Physical TherapistBLS (AHA) Current CO Driver License within 60 days of hire  Medium Work - (Exert up to 50lbs force occasionally, and/or up to 20lbs frequently, and/or up to 10lbs constantly)  This home health role will cover a couple weekend shifts per 3 month schedule period.  There is a 1-0 holidays per year (no Thanksgiving or Christmas).   Your Connected Community:When people and places align, incredible things happen.
