Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Preferred Experience: 1.Fellowship training in Geriatrics. 2.Board Certification in Geriatric Medicine. 3.Greater than two years experience in a management position. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Use of fingers, walking (up to 4 hours), standing (up to 4 hours), ability for rapid mental and muscular coordination simultaneously, far vision correctable in one eye to 20/20 and to 20/40 in the other eye, hearing (aid permitted). Environment Factors, working closely with others, working with hands in water (frequent hand washing), working around moving objects and vehicles, protracted or irregular hours of work. The Deputy Chief of Staff (Geriatrics & Extended Care) duties consist of but are not limited to the following: The Physician will serve as Medical Director of Lake City CLC and will supervise other Physicians, Nurse Practitioners and Physician Assistants in the Lake City CLC. The Physician will also work as an Attending Physician in the Lake City CLC, taking care of Veterans who need Long term nursing home level of care, short term rehabilitation or skilled nursing care in a Nursing Home setting. The Physician will help with admission, discharge, and routine care of these Veterans. He/she will also help coordinate care with other members of the Interdisciplinary Team and with Specialists inside and outside the VA. The Physician will provide education on disease process, medications, and care needs to Veterans, families, and staff. Will participate in screening Veterans for CLC admission. Will also help track outcomes, monitor and report trends to improve quality of care in the CLC. Duties also include supervising and instructing Medical Students, Residents or Geriatric Fellows as needed in an Interdisciplinary Team setting. Will serve as a resource for Veterans, families, and staff for issues related to their stay in the CLC. The Physician may also help with patient care needs in other areas of Geriatrics and Extended Care as needed. Medical Director of Lake City CLC. He/she will work to ensure that quality of care and access to the Lake City CLC residents is maintained. Will help compliance with CMS and VA guidelines on CLC operations. Histories, examinations, assessments and plans for outpatients coordinating all evaluations, diagnostic testing, clinical follow up, and care coordination. Lead the CLC Team in the day to day management of Veterans in the CLC. Promote an atmosphere of efficiency and accuracy in the timeliness of submission of completed documents. Collaborate with Nurse Practitioners/Physician Assistants, ancillary health care providers, and various clinical specialties. Help facilitate hospital admissions, post discharge care and specialty consultations as needed. Participate in screening of Veterans and completion of consults for the CLC. Participate in call schedule for coverage of urgent needs of Veterans in the CLC after administrative hours. Participate in interdisciplinary Team meetings. Assist in developing local policies, templates, and consults to improve provision of quality palliative care, in accordance with VA policy. Act as supervisor for other Lake City CLC providers. Ensure adequate coverage for CLC operations. Submit performance evaluations in a timely manner. Approve leave for Lake City CLC providers. Complete Focused and Ongoing Professional Practice Evaluations semi-annually for staff. Ensure that VA policies regarding access, productivity, and quality care are adhered to. Lead quality improvement activities for the Lake City CLC. Participate in business planning for GEC. Participate in other clinical and administrative duties as assigned by the ACOS for GEC.
