Purpose Statement / Position Summary

The LCSW supporting the Oncology Admin Services department is responsible for developing, implementing, and evaluating oncology support groups and education, coordinating the implementation of the Psychosocial Distress Screening tool, evaluation of tool across disciplines utilizing the tool and tracking and reporting to the Cancer Committee meetings. The position is responsible for the development, implementation, and evaluation of the Survivorship program at MemorialCare Saddleback Medical Center.  This position is an integral part of the Oncology Admin. Services and the interdisciplinary oncology team.

Essential Functions and Responsibilities of the Job

1. Develop, implement, evaluate, and provide support groups for oncology program that includes a Breast Cancer Support Group, Men’s Cancer Support Group and Women’s Cancer Programs. Report data and support group outcomes, from surveys, at quarterly Cancer Committee Meetings.
2. Attend quarterly Cancer Committee Meetings. 
3. Implement Distress Screening Too for cancer patients at each Transition of Care across Saddleback Medical Center (SMC) and Outpatient Services, through coordination with other SMC department and or services
4. Refer patients to various services, contacting physician for order and authorization, as necessary utilizing the Distress Screening Tool to guide referrals to SMC or other services as needed.
5. Responsible for coordinating an Oncology Survivorship program with a multidisciplinary team, that may include rehabilitation services, registered dietitian, oncology nurse navigator, physician(s) palliative care team member and other services, utilizing the American College of Surgeon’s Commission on Cancer Survivorship Program Standard as the guide for program development. 
6. Other duties as assigned.
