ChristianaCare's CareVio team is seeking a Senior Social Worker I who will work in the Wilmington Pediatrics Practice located in Wilmington Hospital, assisting members and families to identify social, emotional, and environmental needs to connect them with available resources/ services through the provision of a full spectrum of discharge planning and social services. This is a Monday- Friday position 8-4:30.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Perform ongoing telephonic case management activities of assessment, problem identification, planning, implementation, coordination, monitoring, and evaluation of case managed members
Collaborates with the members/family, physicians, staff, and other health care professionals across the continuum to provide care coordination and provide the member services throughout the continuum of care
Identifies problems or gaps in community resources that impact outcomes and takes leadership in efforts to effect changes. Coaches members on red flags, triggers, medication management, and followup appointments
Gathers and assesses information regarding the member's physical needs, mental status, family support system, financial resources, and available community and governmental resources to support care plan development
Provides psychosocial support through individual or caregiver option counseling. Continuously reviews service area for group support needs and opportunities and refers to appropriate resources
Manages assigned case load in an efficient and effective manner utilizing time management skills to facilitate the total work process directly monitoring assigned members
Confers with Medical Directors regarding complex cases requiring physician input regarding the treatment plan or physician to treating physician collaboration
Adheres to organizational and departmental policies and procedures and credentialed compliance
Collaborates with other members of the care management team to achieve case management, clinical, operation and financial outcomes
EDUCATION AND EXPERIENCE REQUIREMENTS:
Masters degree in Social Work from an accredited graduate school of Social Work.
Minimum of 2 years' experience related work experience in hospital discharge planning, health plans and or home care preferred.
Training time on the job is three to six months.
