The Department of Clinical Case Management at UC Davis Health provides services to patients and their family members by coordinating the discharge plan for patients who require services, equipment or replacement to meet their ongoing healthcare needs, and completion of clinical reviews submitted to insurance programs to ensure reimbursement for the patients hospital stay.
Minimum of 5 years of case management experience
Minimum of 2 years nursing management experience
Minimum of a Baccalaureate degree in Nursing. In addition, Master's degree in nursing or other related field required or in progress
A valid California Registered Nurse License
Demonstrated expertise in hospital management skills, with progressive expertise in case management, utilization review, discharge planning, home care and/or managed care
Demonstrated expertise in leadership, fiscal, and operations management skills, with the ability to collaborate effectively with physicians, management, and staff at all levels, and external agencies
Expertise competency in financial aspects of healthcare management, blended with an understanding of the ethical, medical, and quality of care issues of case management
Demonstrated expertise knowledge of relevant regulatory requirements, as well as related legislative, accreditation, licensing and compliance.
Certification in Clinical Case Management
Maintains flexible scheduling as required by workload and departmental needs
Rotates with Supervisors to support After Hour and weekend coverage
This position may be subject to a criminal background investigation, drug screen, Live Scan fingerprinting, medical evaluation clearance, and functional capacity assessment.

The Manager of Clinical Case Management (CCM) is responsible for managing and administering all Department of Clinical Case Management programs and operations for UC Davis Health. The manager is involved in policy development, program management, performance improvement, staffing management, department leadership and regulatory requirements, and oversees all inpatients discharge planning and utilization review activities throughout UC Davis Health. The Manager oversees the collection, analysis, and reporting of financial and quality data related to utilization management, quality improvement and performance improvement reporting to the Utilization Review committee. The Manager works closely with the Director of Clinical Case Management and Clinical Social Services and UC Davis Health leadership to develop clearly defined goals and priorities that align with UC Davis Health strategic initiatives. Proactively assesses risk to establish systems and procedures to protect organizational assets. Determines and implements strategies for Clinical Case Management with organization-wide impact. Regularly interacts with executives. Interactions frequently involve special skills, such as negotiating or attempting to influence senior level leaders regarding matters of significance to the organization. The incumbent has a total of 5 Direct FTE with responsibility for a total of 87.2 budgeted full-time equivalents with an annual operating budget of $18,885,724 serving patient populations from pediatrics to adults.
