Work Hours/Shift: Full Time, 12 hour shifts (11am to 11pm) You Will Be Responsible For: - Assigns and facilitates all patient admissions (elective and emergent) and transfers in accordance with established clinical standards and guidelines. This includes acceptance of hospital to hospital transfers, Hospice GIP.
- Maintains bed management software program to allow for accurate patient census lists. Able to produce open bed inventory at moments notice.
- Serves as the central contact person for direct admit patients. Receives physician office calls and facilitates patient placement, ensuring the patient is admitted to the most appropriate bed available using criteria for admission, assigning proper unit, observation or inpatient status.
- Communicates with the ED, Registration, Cardiopulmonary, Patient Care Units, Surgical Services, EVS and other departments to prevent a back-up of admissions and increase throughput.
- Facilitates a daily Huddle and provides length of stay information, admission and discharge numbers. During bed shortage periods, will coordinate additional Huddles as needed.
- Serves as a back up to the staffing office, able to facilitate staffing.
- Review surgical schedule and cath lab schedule for next 24 hours to determine projected bed availability and report to the Director Nursing Operations or designee any challenges.
- Demonstrate tactfulness, diplomacy and courteousness in all contacts with patient, visitors, physicians and any other health team members.
- Integrates service standards in all work relationships.
Qualifications
What You Will Need: - Must be adaptable to a fast pace environment.
- Proven track record showing attention to detail.
- Must be proficient in the English language.
- Pleasant telephone manner and good organizational skills.
- Proficient computer skills and medical terminology required.
- Excellent critical thinking, problem identification and resolution skills.
- Able to interpret EKG rhythms and assist in telemetry
- Previous clinical experience in a hospital setting preferred
- Telemetry, rhythm interpretation preferred
- Familiar with Patient flow, through put preferred
EDUCATION AND EXPERIENCE REQUIRED: - High school graduate or equivalent
- 2 years college preferred
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED: - Certification: CNA required
- LPN preferred
- Paramedic preferred
Job Summary: The function of this job is to facilitate hospital movement by effective bed management which results in effective through put. This individual serves as the facilitator of all patient movement, including admissions, transfers and discharges. The position resides in the nursing operations dept, therefore, the role expands to cover staffing. The role includes assignment of administrative duties and assisting the house supervisor as directed. It is essential to perform this role that this individual understands diagnosis, medical terminology and the individual nursing unit’s scope of care. The role is successful when collaboration with house supervisor occurs regularly and in any time of question about effective patient placement.
