The Registered Nurse - Community Living Center has oversight for admission/approval process for nursing home patients. Responsible for providing Nursing Care to Long Term care residents, dementia unit residents, respite patients, hospice patients, rehabilitation patients, transitional care patients and their families/significant others. Have knowledge and expertise in: Pain management Hospice Care RAI/MDS Accu-max software Meds and Rx via BCMA Geriatric Assessment Skills Care of Dementia Patients Physical Restraint Policy & Procedure Discharge Planning and Teaching Accutech Pt. Wanderer Security System Facilitate Inter-disciplinary Meetings End of Life Care Planning Rehabilitative Nursing Interacting with other therapy modalities, i.e., RT, PT, OT, Restorative Rx. Team building Safety Collaboration Work Schedule: 3:45 p.m. - 12:15 a.m. with rotation to weekends and holidays. Financial Disclosure Report: Not required. Physical Requirements: Heavy lifting, 45 pounds and over; pushing; reaching above shoulders; use of fingers; use of both hands required; walking 8 hours; standing 8 hours; repeated bending; climbing legs only; both legs required; near vision correctable at 13" to 16" to Jaeger 1 to 4; both eyes required; ability to distinguish basic colors; hearing (aid permitted); emotional stability; far vision correctable in one eye to 20/20 and 20/40 in the other. May be required to work with assaultive or combative patients.
