The Lee County Healthcare Center is recruiting for one (1) LPN at their location. LPNs in Primary Care use a care model known as Patient Aligned Care Team (PACT), delivering care to Veterans in the Bay Pines VA and its affiliated Community Based Outpatient Clinics. LPNs have the skills, knowledge, and determination for self-direction and concentrated effort to carry out complex practical nursing care assignments with patients ranging in age from young adulthood to the most senior years. Provides direct nursing care to patients in a variety of healthcare settings, including but not limited to the inpatient arena, primary care, specialty care, community living center, ambulatory specialty care, hospice and mental and behavioral health. Documents care provided observations and responses on appropriate tools or forms, observing changes in patient's condition. Takes vital signs, height and weight, and records in CPRS according to policy and procedure. Identifies, reports, and documents medication errors/discrepancies, reactions and side effects immediately according to policy and procedure. Provides patient education for common disease processes, medications, and treatment regimes. Promotes performance improvement initiatives, identifies problems, collects data, and supports change. Maintains stock levels of supplies and medications. Medication administration including vaccinations Assists with clinic COVID-19 duties including COVID screening and COVID swabs of symptomatic/asymptomatic patients. FIT testing required Other duties as assigned Work Schedule: Extended hours Tuesday, Thursday, Friday, Saturday 730-4; Wednesdays 9-530 for a total of 40 hours.
