The Nursing Assistant at this grade level will function as a member of the nursing care team and assist licensed nursing staff in the care of patients/residents receiving preventive, acute, subacute chronic, maintenance, and hospice care. Position is located in the Community Living Center. Duties include but are not limited to: Obtaining vital signs, documents in CPRS and reporting deviations to appropriate RN. Obtaining Intake and Outputs appropriately (oral intake, IV pump, feeding pump, Foley catheter, urinal, drains, etc.) and reporting shift totals/deviations to RN. Gathering information about patient condition and communicating to RN to develop specific plan of care for patient. Recognizing subtle changes in the patient's condition, behavior and/or comfort. Reporting observations and interventions to RN. Providing environmental safety, security, infection control and patient privacy. Demonstrating ability to plan and organize assignment independently. Performing and documenting Q2 hour rounding/turning. Verbalizing and demonstrating process for obtaining lab specimens, proper labeling and immediate transport. Recognizing and responding to impaired airway-notifying RN. Providing oxygen as ordered or as RN instructs-using the appropriate equipment. Providing care for & management of patient with chest tubes. Performing 12-Lead ECG with correct lead placement. Verbalizing and demonstrating the process for retrieval and placement of the telemetry unit. Obtaining urine output (i.e., bladder scan, I&O documentation, etc.) and reporting finding to appropriate RN. Providing Foley catheter care each shift, daily or as needed. Verbalizing process in notifying RN regarding changes in patient skin status. Verbalizing rationale when performing Q2 hour rounding/turning requirements. Demonstrating proper hand hygiene prior to and immediately following patient care/contact. Work Schedule: Rotating Tours of Duty based on the needs of the unit to include days, weekends, evening and night shifts. Primary shift schedules are: 7:30 a.m. - 4:00 p.m.; 3:30 p.m. - 12am; and 11:30 a.m. - 8:00 p.m. Other tours of duty that may be utilized are: 5:00 a.m. - 1:30 p.m.; 6:00 a.m. - 2:30 p.m.; 9:00 a.m. - 5:30 p.m.; and 1:00 p.m. - 9:30 p.m. Telework: Not Available Virtual: This is not a virtual position.
