Fairview Health Services' Palliative Care Program has an exciting opportunity for a LICSW Social Worker for St. Joe's (St. Paul) and St. John's (Maplewood) Hospitals. Fairviews Palliative Care Program delivers state of the art palliative care through specialized interdisciplinary teams while improving the quality of care for patients and their families throughout the Fairview system. Palliative Care staff work with the patient and their health care team to help relieve physical, spiritual and emotional suffering. Care is centered on the unique needs of each patient. It seeks to promote the highest quality of life and comfort while honoring the traditions and beliefs of the patient and family.

The practitioner provides a comprehensive, and specialized range of clinical social work practice to assist individuals, families, couples and/or groups to restore, enhance, or protect their capacity for productive social functioning and coping with the consequences of their acute or chronic illness. The individual assumes a leadership role in developing interventions that improve health care delivery to specific patient populations, quality improvement, and professional practice areas. The individual assists other staff in their development through clinical consultation, education and evaluation of professional practice. The position requires a high level of independent clinical practice, functioning and leadership within the program, service area and department.

This is a full time, 1.0 FTE, salaried, day shift (no weekends), benefits-eligible position splitting time between St. John's Hospital in Maplewood, MN and St. Joe's in St. Paul, MN.

• Provides a range of clinical social work interventions that include advocacy, casework, counseling, crisis intervention, and care transitions management in assisting patients and families to reach their goals.
• Plans and implements interventions and transition planning based upon psychosocial and risk assessments that are integrated into the patients plan of care.
• Initiates/implements transition functions and activities for patients, communicating with patients, families and the health care team to ensure seamless transitions. Determine if caregiver training is needed.
• Arranges and coordinates a comprehensive range of community resources; maintains up-to-date knowledge regarding resources and current financial information.
• Takes a leadership role in facilitation of interdisciplinary communication, guiding interdisciplinary team member interactions with patient/families experiencing complex psychosocial issues.
• Identifies and initiates appropriate system change to enhance patient and family satisfaction.
• Effectively communicate transition plans across the continuum of care.
• Partners with inpatient/outpatient care team on patient care issues.
• Takes leadership in revising or restructuring program or service standards as needs change..
• Acts as social work consultant to various levels of consultees to problem-solve complex patient care situation.Mentors internal members of the health care team on care transitions concepts. Facilitates integration of concepts into daily practice.
• Understands and focuses on key performance indicators. Actively tracks outcome and participates in quality planning

Education:

• Masters degree in Social Work from an accredited social work program.

Experience:

• 3+ years experience in a health care setting, demonstrated ability and competency to analyze, apply, intervene, and evaluate a specialized range of psychosocial theory and practice models with clients. Also, demonstrates a comprehensive knowledge of community and economic resources in the area of specialization.

Skills:

• Ability to skillfully interact, assess, diagnose and intervene with patients of all ages and degrees of disability, families, physicians, other health providers, and the community.
• Excellent verbal and written communication skills.
• Ability to synthesize, diagnose and apply a specialized range of psychosocial knowledge and practice interventions with patients in a specialty, clinical service, or ambulatory care.

Licensure:

• Current Minnesota Social Work License at the LICSW level.
