The role of a hospice social worker is to advocate for each patient's end-of-life wishes and help individuals address the emotional aspects of late-stage illnesses. Hospice social workers also assist families in identifying other available local services and resources for additional support.
This position will provide supportive and medical social services to terminally ill patients and their families in accordance with an appropriate plan of care. The social worker will assume responsibility for the development, implementation, and management of psychosocial services provided to hospice patients and families in the home care or residential setting.
Masters in Social Work / Must also have the LSW
