This is a full-time professional position. This position is responsible for the management and quality of care provided in the community-based hospice program, including nursing, support services, Certified Nursing Assistants, social workers, and ancillary therapies for in-home hospice care. This position supervises the Community-Based Hospice Team Leaders and Clinical Manager of Patient Access. This position, in coordination with the Vice President of Clinical Services and Vice President of Quality and Compliance, is responsible for assuring the agency is in full compliance with all regulatory and quality of care requirements and accepted standards of practice. This position is a member of the Clinical Leadership Team and works with the Vice President of Clinical Services to provide management oversight and strategic planning for the agency in-home hospice services. The schedule for this position is Monday-Friday 8:30am-5:00pm.
Responsibilitieswhat you do
Manage the In-Home Hospice care program in compliance with NHPCO quality standards of practice, ethical principles and strategic goals of Transitions LifeCare. Ensure compliance with legal and regulatory requirements.
Ensure the quality delivery of interdisciplinary services is available 24 hours a day 7 days a week. Ensure collaboration, communication, timely response, and continuity of care across all disciplines and settings.
Promote exceptional customer service.
Supervise In -Home Hospice Care, Clinical Management with an emphasis on equipping, training, coaching and motivating Clinical Management. Provide oversight of contracted services, pharmacy, DME and therapies. In collaboration with Director of Education, plan education and training activities.
Coordinate the activities if Interdisciplinary Groups (IDGs) with one another, and with facilities and Hospice Home staff.
Accountable for the effective and efficient operation of the interdisciplinary clinical staff, including ensuring that orders, treatments and plans of care are being carried out in a manner supportive of patient and family goals.
Regularly review and revise Community-Based Hospice policies and procedures as needed, including referral and admission policies, determining levels of care, and clinical pathways for care planning.
Review transfers, revocations and discharges with the Chief Medical Officer.
Manage the Community-Based Hospice programs budget and develop team budgets in collaboration with the Vice President of Clinical Services.
Ensure adequate, trained, diverse and competent staff and volunteers are available to meet program goals and patient and family needs. Oversee utilization of resources across teams.
Ensure that documentation of patient care is complete, timely, and accurate in accordance with TL processes, policies, procedures and regulatory requirements.
Analyze, develop and maintain an efficient and effective nursing care delivery system that meets patient and family needs across the continuum.
Actively support and promote a professional practice model that encourages staff and Team Leader participation in the development of Clinical Standards that is collaborative and utilizes current trends and data in professional practice.
Participate in strategic planning activities. Design and evaluate processes and services to ensure Community-Based Hospice strategic goals and clinical patient centered goals are achieved.
Maintain positive relationships and open communication with SNFs and ALFs in service area.
Participate in performance improvement activities and in the annual evaluation of the Community-Based Hospice program and Teams, including patient care outcomes, satisfaction surveys, productivity, patient census and financial goals.
Assist with timely customer complaint follow up, resolution and documentation.
Participate in community education and outreach activities.
Manage various personnel actions including, but not limited to, hiring, orientation, performance appraisals, promotions and scheduling; and counsel and conduct formal discipline of direct reports and associated clinical staff.
Collaborate with Clinical Managers and Directors across service lines to provide patient/family centered care.
Maintain required records, reports and statistics for administrative purposes, ensures compliance with established hospice policies, procedures and objectives, including, quality improvement, PI, safety, environmental and infection control. Communicate requirements to staff and hold staff accountable to established requirements.
Oversee the Hospice Mentor and Preceptor Programs.
Perform other duties as assigned by Vice President of Clinical Services.

Requirements what you need
RN with current unrestricted N.C. license. BSN required, Masters Degree in Nursing or Healthcare related field and Hospice Nurse Certification preferred.
Requires an in depth knowledge of hospice and health care organizations and their functional relationship.
Proven ability to manage projects and relationships, which extend beyond the Transitions LifeCare structure.
Thorough knowledge of reimbursement, certification and accreditation issues.
Innovative, able to communicate effectively and trained/experienced in current management practices.
Proven ability to develop and educate staff and community.
Must be able to function professionally in the work place and other health care settings as appropriate to the need.
Must have sensory abilities to complete communication and documentation requirements of the job.
May be required to work for extended hours in front of a video display terminal and use repetitive motions.
