This is a full-time professional position that will provide case management for home health patients assigned; providing optimum patient comfort as part of an interdisciplinary team under that attending physician’s orders consistent with the agency’s mission statement and regulatory systems. The schedule for this position is Monday-Friday 8:30am-5:00pm.
Responsibilities what you do
Provide nursing care for assigned patients as indicated under the Physician’s plan of treatment
Coordinate the Interdisciplinary Care Team for assigned patients
Teach primary caregiver skilled needed to care for the patient in their residence
Participate in Interdisciplinary Team Meetings
Assist with assessments and admission of patients to services under the attending physician orders
Document inpatient medical record to meet federal, state, regulatory and agency guidelines
Conduct chart audits, QA/UR, specified studies, and assist in the office as assigned

Requirements what you need
Registered Nurse with current NC license; BSN preferred
Minimum 3 years recent clinical experience; Minimum 2 years home health and/or hospice experience or equivalent
Knowledgeable about reimbursement source, Medicare regulations, licensing laws, and accreditation standards
Computer skills preferred
Must have access to transportation.
Current CPR certification.
Must be able to travel to, enter and function in the homes of any patients/family in the service area to assess and provide care to confined patients
May be required to lifts and /or carry items up to 50 pounds
Must have sensory abilities to complete physical assessment, communicate with patient /family and physician and to complete the reporting and follow-up documentation responsibilities
