Job Summary
This is a full-time professional position.  This position is responsible for completing the admission, initial assessment and associated documentation for new home health patients, as well as conducting RN field visits whether scheduled or PRN.  Duties will be performed in a manner that is consistent with the agency’s mission statement, core values and regulatory requirements. The schedule for this position is Friday-Monday 9:00am-7:30pm.
 
Responsibilities what you do
  • Utilizing expert level clinical decision making, assess and develop patient plan of care for all elective, urgent and emergent admissions.  Requires expert level analysis of patient clinical information and formulation of an accurate assessment of the patient and his/her condition.
  • Plan, provide, supervise and document safe, competent, and compassionate nursing care for patients in accordance with physician orders and established policies and procedures.
  • Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings
  • Responsible for providing and documenting appropriate and relevant information and education to patients and their families in compliance with current Centers for Medicare and Medicaid Services CMS Conditions of Participations (COPs) for Home Health.
  • Participate in the identification of clinical or operational performance improvement opportunities and in performance improvement activities.
  • Responsible for professionally managing time, people and technology.
  • Monitors and manages the appropriate use of patients’ medical equipment, supplies and medications.
  • Works collaboratively with Interdisciplinary Care Team members to coordinate care for the patient.
  • Provide coverage for nurse case managers for Home Health.
  • Use professional nursing judgment to individualize the plan of care based on assessment of the patient’s unique needs and response to care.
  • Delegate tasks and supervises the activities of LPNs and Certified Nursing Assistants, as needed.
  • Responsible for following current accreditation standards and CoPs for Home Health as related to Certified Nursing Assistant (CNA) supervision and LPN supervision.
  • Rotate Holiday On-Call with Home Health Staff
  • Perform other duties as assigned by supervisor

Requirements what you need
  • Registered Nurse with current NC license; BSN preferred
  • Minimum 3 years recent clinical experience; Minimum 2 years home health and/or hospice experience or equivalent.
  • Knowledgeable about reimbursement source, Medicare regulations, licensing laws, and accreditation standards
  • Computer skills preferred
  • Current CPR certification.
  • Must have access to transportation
  • Must be able to travel to, enter and function in the homes of any patients/family in the service area to assess and provide care to confined patients
  • May be required to lifts and /or carry items up to 50 pounds
  • Must have sensory abilities to complete physical assessment, communicate with patient /family and physician and to complete the reporting and follow-up documentation responsibilities of the job
