The Inpatient Community Living Center (CLC) and Palliative Care Clinical Pharmacist is a licensed professional with knowledge and proficiency in pharmacy practice, who is responsible for medication therapy management; from admission through discharge. The position works with the interdisciplinary team and through clinical assessment, assesses patient responses to medication therapy, makes appropriate recommendations, and implements and monitors pharmacotherapeutic care plans.
