A full-time (40 hours per week) Monday-Friday 8a-5p Human Services Position is available.
Candidate will supervise all social work, chaplains, and bereavement staff and coordinate all functions of social work, chaplain, and bereavement care provided to hospice patients. Primary responsibilities to include management of the social work team, psychosocial assessments, counseling, and development and implementation of the plan of care related to psychosocial needs.
This position will also be responsible for the supervision of social work staff and Chaplain Team Lead and will include new patient assignments, annual performance evaluation and corrective action as needed. Duties will also include selection, hiring, orientation, and training of social workers, chaplains, and bereavement staff with help from the Chaplain Team Lead.
The Human Services Manager will ensure representation of these teams in IDG meetings, be an active member of the Administrator On-Call Team, and a contributing member of the Leadership Team as a representative of the social work, chaplain, and bereavement departments. This candidate will also champion the organization's We Honor Veteran's program.
Master's Degree (MSW) as well as an LCSW with experience
