Help Deliver the Best Home Health Care to Our Community as a Team Leader, Clinical Nurse
As a Team Leader, Clinical Nurse you are responsible for the coordination of the departmental clinical operations for home health with primary focus on home visits. You will be part of the Home Health Department which provides full-service home health to patients in Wake County and some of the surrounding counties. Further responsibilities include:
Coordinate admissions/referrals to facilitate timely patient flow throughout the department.
Delegate patient care to appropriate personnel by making assignments based on needs, abilities, and expertise.
Understand the needs of the organization and support the mission, values, and management of patient-care services.

Graduate in Nursing Required
Bachelor’s Degree in Nursing Preferred
2 Years of Clinical Experience in Home or Community Health Required
1 Year of Leadership Experience Preferred
RN License Required
