Assist in managing the department’s operations related to Community Care specific to the Home Care Department. Assists the Clinical Manager in supervising and providing skills competency oversight to all caregivers. In collaboration with the Director and the Managers, the Supervisor ensures that quality professional and nonprofessional services are provided to patients at the level that the patient requires

Assists in managing department operations to insure framework for optimal patient care during patient’s home care stay in coordination with appropriate recuperative needs and safety for discharge to next provider.
Assist in managing the nursing, aides and therapy staff utilization management functions such as providing supervisory visits, providing feedback on performance, coaching and teaching. Works closely with the Clinical Managers by responding to reports, audits and any denial of payment from external payers and reviewers as requested. Insures the home care service maintains or exceeds the expectations of external payers and reviewers.
In collaboration, assists managers in selecting and training Caregivers for all positions stated above. Trains, supervises, counsels, and evaluates all caregivers as requested by Clinical Manager/Director.
Assists in scheduling caregivers to optimize efficiency and effectiveness. Provides caregivers with support, encouragement and motivation to perform job duties in optimal manner.
Assists in development and helps maintain department policies and assists in development and maintenance of policies related to general department operations, discharge planning and patient care.
Participates in department Performance Improvement, Corporate Compliance, Patient Satisfaction, Health Team Satisfaction and Caregiver Engagement initiatives and monthly reporting of key operational information. Identifies opportunities for improvement. Plans and implements strategies to improve department operations and patient outcomes consistent with Sparrow Health System Plan of Excellence.
Provides education to department staff, physicians, other hospital departments and others related to department operations and responsibilities.
Assists in the development of department budget, goals and objectives. Helps implement plans to achieve goals. Participates in efforts to support and advance the mission, vision, strategies and goals of the Health System.
Monitors customer satisfaction, performance improvement and corporate compliance indicators, and key indicators of effectiveness and efficiency to ensure optimal performance.
Participates in establishing policies and procedures related to position descriptions, staffing needs and productivity.
Assists in the development and implementation of special projects.
Assists in clarifying physician’s orders, obtaining verbal/telephone orders and documenting these orders for patients in Home Care.
Maintains proficiency in computerized documentation practices and can mentor others as they learn software documentation.
Current Michigan Nursing License. BCLS. Demonstrates excellent case management knowledge through certification. If not already certified when assuming this position, Home Care OASIS certification will be required within three years as a condition of continued employment in the Supervisor position. Active, valid State of Michigan Driver’s License. Active car insurance and able to produce proof of same upon request.
Minimum of two years nursing experience in an acute care hospital. Minimum of five years nursing experience in Home Care. Minimum two years of leadership experience. Experience in completing OASIS assessment and discharge planning preferred Experience with McKesson Home Care and McKesson Scheduling software programs preferred
Graduate of an accredited Nursing program. BSN or Bachelor’s degree in related field preferred
