JOB TITLE: Registered Nurse Navigator Palliative Medicine

The Palliative Medicine Nurse Navigator (PMNN) provides psychosocial and medical management services to patients and their family members as well as representing the department in community activities.
Palliative medicine patient and family symptomatic, psychosocial and spiritual needs are complex and require a high degree of clinical oversight and creative problem-solving. The PMNN manages, plans, implements, monitors, and evaluates the delivery of services within the Palliative Care Program. The PMNN should be expert in identifying available social work services to hospice and palliative care patients and has knowledge of hospice and palliative care programs. The PMNN will participate in other special duties and projects as assigned by the Director of Palliative Medicine Services. The PMNN will interface with other managers and medical services to assist in identifying appropriate patients for a palliative medicine consult.
Performs duties to support the goals and objectives of the Geisinger Health System in its efforts to provide quality patient care and the highest level of patient experience.
Works under the administrative direction of the operations manager and the clinical direction of the Palliative Medicine physician director.
COMPETENCIES AND SKILLS:
Demonstrates effectiveness and accuracy in completing responsibilities.
Demonstrates ability to cooperate with the physicians, nurses, receptionists, secretaries, and ancillary personnel in assuring a smooth coordinated effort in delivering quality patient care.
Demonstrates ability to work in a team environment.
Demonstrates ability to deal directly with patients in such a way as to instill confidence and facilitate communication.
Demonstrates a high-level of critical thinking and communication skills.
Computer literacy required. Preferred applications: MS office, Outlook, EPIC
EDUCATION AND/OR EXPERIENCE:
Bachelor's degree in Nursing or equivalent, with current Pennsylvania licensure, required. Master's degree preferred.
Minimum three to five years clinical experience required.
Experience with palliative medicine preferred.
BLS certification required.
